Care Planning Assessment Tool (CPAT)

CPAT is a holistic assessment tool. It captures information about a person's need and ability in eight areas of care

These areas of care are:

  • Communication
  • Physical
  • Self-Help
  • Confusion
  • Behaviour
  • Social
  • Psychiatric
  • Carer dependancy

The CPAT program recognises that the care worker knows the person the best, and therefore empowers the care worker to take charge of the case-management processes to create an accurate visual summary of care and provide reports that:

  • show the person's care needs and abilities in relation care-service levels and to show how needs change over time

It also empowers the care worker to initiate and support discussions with multidisciplinary team members, family members or other relevant persons to improve the care and service.

Secondary advantages

CPAT provides a common context to help professional staff to communicate from the same frame of reference and allows service managers to combine individual assessments to create a service profile that defines the focus of the total services they provide.
