The CPAT program recognises that the care worker knows the person the best, and therefore empowers the care worker to take charge of the case-management processes:

  • To create an accurate visual summary of care
  • Provide reports that:
    • show the person's care needs and abilities in relation care-service levels and to show how needs change over time
